For me, it happens when I wake up and realize I’ve been hard all night and it’s extremely painful. Too much flow in, not enough out. It can be so painful that I’ve taken an ice bath hoping it would go down. This is last summer when I had a pretty distinct discolouration following a few powerful episode. It’s just iron in the skin and has since disappeared (mostly). Can be a bit scary to have an erection that is so powerful and lasts for hours that is against your will, cannot be changed by anything. I’ve been to the WR a few times but never had any need for aspiration as it’s never lasted more than 4h after waking up. On the plus side though I know that my adult growth from big to enormous penis is (at least partially) thanks to that.
This video shows the discolouration pretty well. It’s just in the skin, purely just funny looking and not indicative of anything dangerous.
